To enroll as a member in the PersonalMDs membership practice:
Please call or email the office to schedule a personal meeting with one of our physicians. Our office manager would be happy to send you an enrollment package in advance if you wish.

Fees
The yearly fees are as follows:
Individual membership: $3,600
Couples: $6,000 (Children age 17 to 26 included at no extra charge)
Single Parent with children age 17 to 26: $4,000
